TO DOWNLOAD REGISTRATION FORM:
     - Place curser on registration form below
     - Right click on mouse
     - Go to "Print Picture" and left click

   *** IF THAT DOESN'T WORK:
     - Place curser on registration form below
     - Right click on mouse
     - Go to "Save Picture As..." and left click
     - Choose where you want to save the registration form on your computer
     - Go to where it is saved on your computer and open up the document
     - Under "File" at top left click "Print"
     - (If you do not see "File" just place your curser on the open document, right click then left click on "Print")
